| For your convenience we have provided our health history and patient registration forms. Please print and fill them out in advance. Bring the completed forms on the day of your appointment to expedite the check-in process. If you do not have the forms completed in advance you should arrive 15 minutes earlier for your appointment. |

Please bring the following with you:
- Insurance card
- Paper referral, if needed
- List of current medications, including OTC and herbal supplements
- Your co-pay
If you are undergoing full-skin examination, please do not wear any makeup, hair spray, or nail polish. However, these can be applied before you leave.
If you cannot keep your appointment, please provide notice 24 hours in advance. Also, please remember to arrange for your referral if it is required. |
